It is not known if this issue was provided with a dustjacket.

Notes:

The exact nature of the flexible binding of Chivalry (and of The Line of Love) is not known. It has been suggested that these are remainder bindings, but it seems more likely that they were advance copies, as the binding and presentation more resembles an in-house reading or review copy than an issue for general sale.  Not being bound in boards, and with all edges trimmed, the size is about 1-cm (0.4 in) shorter and the same amount narrower than the 1st, 2nd and 3rd states. It's also worth noting that copies of this issue are not uniform in size - examples have been noted with front cover dimensions varying from 211 x 138-mm to 214 x 142-mm.

The frontispiece and eight of the color plates are by Howard Pyle; two are by William Hurd Lawrence; and one is by Elizabeth Shippen Green.
